Community Care Ministries provides non-emergent primary care, chronic disease management, medication assistance, assistance with specialty referrals, limited mental health counseling, and foot care.

We provide these routine medical appointments free of charge to eligible individuals meeting the following requirements in Pottawatomie, Wabaunsee, and surrounding counties.

• Uninsured: having no health benefit (No Medicare, Medicaid, Tricare or Private)

• Household income: must be at or below 200% of the federal poverty guideline
