CCM provides quality healthcare by appointment only. These appointments are free of charge for those without health insurance and income is at or below 200% of Federal Poverty Level. Through the generosity of donors, we are able to provide these services.

Community Care Ministries requires the following before ANY services can be provided:

Proof of income for all working members of the household: Please provide your three (3) most recent paystubs.

·Complete Application
·Medical Screening
·Social Screening
